FEES

Individual Therapy: $125 per 50 minute session

Military (Active/Retired/Veteran/Spouse): $110 per 50 minute session

Full-time Ministry: $110 per 50 minute session

INSURANCE ACCEPTED

Tricare East (Select, For Life, Reserve Select, Retired Reserve, and Young Adult )

Prime - I am an Out of Network (OON) provider, although a Certified Tricare therapist, you can use a Point Of Service (POS) option which includes a deductible and then 50% session fee each session thereafter.

All Other Insurance Providers: I am considered an Out of Network Provider. I will provide you the form CMS 1500 which you submit to your insurance company (subject to your coverage) for reimbursement.*

If you have a PPO insurance plan and your health insurance company has authorized you to see an "out of network provider", you may use these receipts to request reimbursement from your insurance provider. Please contact your insurance company to determine if this is an option, what percentage of money you spend will be reimbursed, and to clarify your process for obtaining reimbursement.  Please check your coverage carefully to verify specifics of your particular insurance coverage and ask the following questions:

·     Do I have mental health benefits?

. Does my plan cover out-of-network providers? 

·     If so, what are my out of pocket expenses?

Payment & Cancellation Policy

It is the responsibility of the client to provide payment at the time of service. I accept all major credit cards and debit cards as forms of payment (with a 3% processing fee).  

Cancellations less than 24 hours from your scheduled appointment will be charged the full session fee.  Please schedule carefully.
